MTC APPLICATION FOR ADMISSION

To submit an application to our school, please complete the following form and select Submit Application.

• A valid Social Security Number (SSN) or Taxpayer Identification Number is vital information MTC uses to comply with federal and state reporting requirements. In order for you to be able to access your MTC email and courses through Canvas, you will need to provide your SSN. If this info is missing on your application, you will need to submit a "Missing SSN form” by fax or US mail to the Office of Student Records. This form can be located from your school counselor, or requested by calling MTC directly at 740-725-4002 or 740-386-4104.

    College Credit Plus Permission

    We, the undersigned, hereby understand that by enrolling in College Credit Plus courses:

    Content may include mature subject matter that will not be modified based upon College Credit Plus enrollee participation regardless of where course instruction occurs

    State law requires this signed form be submitted in the student's application to the college or university following that college or univerity's instructions for submission of application materials

    I certify to the best of my knowledge that the information herein is true. I authorize all previous schools attended to release my academic records for the purpose of enrolling at Marion Technical College. By submitting and dating this application, I agree to abide by the policies and regulations of the College as published in the MTC catalog. The criteria for admission to Marion Technical College also requires high school transcripts as well as college transcripts (if applicable). Students should request that these be sent to the Office of Admission, 1467 Mt. Vernon Ave., Marion, Ohio 43302. Providing your social security number (SSN) is voluntary. Your SSN is not used as your student ID number. We request your SSN for compliance with federal and state reporting requirements. Supplying a SSN is necessary for students to qualify for educational tax credits, enables certification of Veterans benefits, and enables the College to provide information to government entities that require SSN, including but not limited to the federal government for financial aid and Tax Relief Act(1997) reporting, Immigration and Naturalization Service, and Ohio Board of Regents. Marion Technical College will not disclose your SSN without your consent for any other purposes except as allowed by law.
